Forms

The Registrar's Office has several forms available online.
If the form you need is available, please print it, fill it out, obtain appropriate signatures, and return it to the Registrar's Office in North Chesapeake.

Attention Students:  The Office of the Registrar strictly follows University Guidelines for the Honor Code.  Please be aware of this when filling out any of the forms listed below.

PDFAdvancement to Candidacy Used for Doctoral students to be advanced to candidacy
Academic Scheduling Office Forms for departmental academic scheduling coordinators.
PDFBachelor's/Accelerated Master's Transition Used with the bachelor's graduation application only for those accepted into an accelerated master's program.
PDFChange/Declaration of Academic Program (Undergraduate) Used to change an Undergraduate catalog year, declare/change a major, major option (concentration), minor, or certificate program.
PDFCourse Audit Must be submitted to the Registrar's Office by the last day to drop.
PDFCredit Reassignment Request Used for current Undergraduate Degree seeking students to transfer previous Mason Non-Degree work into their current Undergraduate program.
PDFCredit without Grade (satisfactory/no credit) Must be submitted to the Registrar's Office by the last day to add.
PDFDoctoral Dissertation Research Grace Period for Full-Time Equivalent Status.
Domicile Services Forms to apply for in-state tuition privileges.
PDFGraduate Assistant Full Time Status Request For Graduate Students with assistantships of 20 hours a week and a minimum of 6 credit hours for the semester.
PDFGraduate Course for Undergraduate Credit Must be submitted to the Registrar's Office by the last day to add.
PDFGraduate Student Request for Undergraduate Course Registration Used by Graduate Students who wish to register an Undergraduate Course. Must be to the Registrar's Office by the last day to add.
PDFGraduate or Non-degree Graduate Re-enrollment Students must also complete the Application for Virginia In-State Tuition Rates (page two of this form) in order to be considered for in-state tuition rates when re-enrolling.
PDFGraduate Transfer of Credit Request Used for Graduate students to transfer credit from previous institutions attended or from Mason Non-Degree
Graduation Forms Forms from the Graduation Office.
PDFIncomplete Extension Petition for extending the deadline for submission of completed work.
PDFIndividualized Section Must be submitted to the Registrar's Office by the last day to add.
PDFMaster's Thesis Research Grace Period for Full-Time Equivalent Status.
PDFNon-degree Undergraduate Re-enrollment New non-degree studies students must fill out the Non-Degree Studies Application through the Office of Admissions.
PDFNon-Credit Record Request Permanent record of Mason-sponsored, non-credit learning activities. May include workshops, seminars, and certificate programs.
Office of Student Accounts Forms for tuition refunds and payments.
PDFPermission for Undergraduate to take Reserve Graduate Credit Must be submitted to the Registrar's Office by the last day to add.
PDFSelective Withdrawal Request For Undergraduate, Degree seeking students. (Formerly Elective Withdrawal)
PDFSenior Citizen Tuition Waiver Senior citizens must first be admitted through the Non-degree Office before they may register.

PDFStudy Elsewhere Must be submitted to the Registrar's Office prior to the taking of the course at another institution.
PDFTranscript Request Used to request a transcript.
PDFTransfer Course Equivalency and Substitution/Waiver Forms

Used for course equivalencies, transfer course re-evaluations, and/or substitutions/waivers of degree requirements.

Veterans Registration Reporting  
PDFVerifications Request Allow 5 days for processing.
PDFVoluntary Resignation from Graduate Academic Program Students who have been granted a resignation will not be able to register for any courses unless admitted to another degree program or non-degree status in a different program.
